Skokie Community Health Survey – Your Health – Your Community- Your Voice
Skokie’s Community Health Survey helps the Health and Human Services Department gain insight into factors that may impact the community's current health and how they may be improved. The data collected will help a group of community leaders set goals and assist us in designing and implementing the Community Health Improvement Plan (CHIP).
We are asking Skokie residents to participate in a short and anonymous Community Health Survey. The data not only helps us know how we can improve the Village of Skokie, but it gives you a chance to express what matters to you. Identifying topics that are important to you will help to shape future planning in Skokie. Your voice matters. To take this survey use this link: https://www.surveymonkey.com/r/Skokie2022. The survey is open through September 30, 2022, will take about five to seven minutes to complete, is anonymous, and available in English, اردو, Española, Tagalog, فارسی and Русский.
As part of the community health assessment process, we are looking for Skokie residents to participate in a community health meeting scheduled for November 18.
