Birth & Death Records

The Skokie Health Department has served as the local registrar for all births and deaths that have occurred within the Village since 1969. To receive certified copies, the entitled person can do so in person or by mail.

Applications


Birth Records


$10 for the first certified copy of a birth record and $5 for each additional copy at time of purchase. View Birth Certificate Application Form (PDF).  Birth Certificates can only be purchased by listed parent or child listed on the birth certificate if they are 18 and older.

Death Records


$12 for the first certified copy of a death record and $7 for each additional copy at time of purchase. View Death Certificate Application Form (PDF).

Payment


Check or money order for the fee payable to the Skokie Health Department. Visa, Mastercard and/or Discover credit card information if not using check or money order for payment. The Requester’s signature and relationship to the child or deceased / estate.

To obtain a certified copy of a birth/death record in person visit:

Monday through Friday between 9 - 11:30 a.m. or 1:30 - 5 p.m.  Individuals will be asked to produce valid photo identification at the time of purchase.

Required Information


Required information for Birth Records if mailing without a printed form:
  • Child’s complete name as it appears on the certificate
  • Child’s date of birth
  • Child’s mother’s maiden name
  • Child’s father’s name if it appears on the certificate
  • Include a phone number
  • Include a mailing address
  • Include a legible, clear and large print photocopy of a valid drivers license or state ID of the person requesting the certificate
Required information for Death Records if mailing without a printed form:
  • Deceased name as it appears on the certificate
  • Deceased date of death
  • Include a phone number
  • Include a mailing address
  • Include a legible, clear and large print photocopy of a valid drivers license or state ID of the person requesting the certificate.  *If Funeral Home, please also include the Funeral Directors License information
  • Shipping and handling fees may be applied.  Call the Health Department and ask for the rates
